This is the primary submission form for Anime USA panels and workshops (collectively, "sessions"). Submissions must be in no later than 30 August 2008; any other submissions after this date will be approved only on a case by case basis. No sessions will be accepted at all after September 27, 2008.

Only one session per submission. If you have additional panels you wish to submit, please fill out this form again.

For those of you who don't wish to use the form, you can also email the Programming department at programming@animeusa.org with the same information as listed below.

Name This must be the name of the primary contact; please do not use a nickname or screen name. If you wish to use a nickname in your session description or during the session, please inform so in the comments section. If there is more than one person running the session, please list additional names in comments.

Email This must be the email address of the primary contact.

Phone Number This must be the phone number of the primary contact.

Subject This will be the title of your session. If your panel has an overly long title (i.e., for comedic effect), please create a short version and place here and the long version in the comments.

Session Type Please check the type of session you will be running. Please be aware of the following:
1. Multimedia sessions (e.g. Jrock music sessions) may be limited due to noise, equipment and time constraints.
2. Due to the nature of Otaku Blue sessions, no one under 18 (this includes session participants) will be allowed to participate in an Otaku Blue session.

Multimedia - Music or Video-based sessions (e.g. Jrock listening session)

Block of Time Required Please check the amount of time your panel or workshop will need; the programming staff will do its best to accommodate your request.

Please bear in mind that the 15 minutes in between sessions will be used for setting up by the following session.

Description Enter a short description of your panel and what it is about. This description should be no longer than two paragraphs and may be edited for clarity and length. It must be at least 100 characters long.

Comments For your panel, we require the following:

1. If your panel is an Otaku Blue panel, we require the names and ages of each participant as of October 10, 2008 (the starting day of the convention). We will be checking ages via ID prior to the start of each Otaku Blue session.

2. Anything mentioned above that does not fit in the standard columns or any other questions you may have.

Technology Support Please check the type of tech support you will need; more than one box may be checked. Please be realistic and do not select EVERY box. If no logistics are required, please do not check.

If you require something that is not on the list, please note it in the comments section above.

Final determination of support will be on the basis of the AUSA mobile tech division.

AVD - Television with VCR and/or DVD capabilities
BEM - Easel with paper ("butcherboard") and markers
MAC - Connections for Apple-based laptops
MAS - Microphone audio setup (note: this support is limited)
PGP - Paper and golf pencils for audience (e.g. for drawing workshops)
PPT - Display projector for slideshow ("PowerPoint") presentations
